Rachel Slack, P.T., D.P.T.

Rachel was born and raised in Phoenix, Arizona. She earned her bachelor’s degree from MidAmerica Nazarene University in Olathe, Kansas, and then went on to complete her Doctor of Physical Therapy degree from Rockhurst University in Kansas City, Missouri.

Rachel has clinic experience in both skilled nursing and outpatient orthopedics. The Phoenix native played club soccer for SC del Sol and also received physical therapy and sports performance training as a former OrthoArizona patient. She went on to play four years of NAIA collegiate soccer.

Outside of the office, Rachel enjoys painting, drawing, and going to First Friday. She stays active by hiking and playing adult league indoor soccer.
