Sports Physical Program
As part of our ongoing effort to provide collegiate-level care for high school athletes in the East Valley, OrthoArizona invites you to attend one of the sports physicals being held select weekends in April and May. Join your teammates at your school and have your sports physical completed. Your school representatives will be on hand to help so you are prepared and ready to go for your upcoming athletic season. This physical is good for all athletic seasons during each year.
The cost of the physical is $25 (cash only) and referrals to subspecialty doctors will be included in the cost. Proceeds benefit Gilbert Public Schools and Chandler Unified School District High School Sports Medicine Program.
If you are planning on attending the event please have a parent or guardian with you or complete the consent forms below. In addition, you must have the AIA physical form completed prior to your visit.
