Patient Consultation
Our Sports Medicine Center at OrthoArizona offers patient consultations Monday through Friday.
Prior to consultation, new patients will need to complete forms covering medical history, demographic details, and insurance information. Our doctors will use this information, along with any current diagnostic imaging (X-ray, CT, MRI), to develop a diagnosis and treatment plan.
